PO-02-029 USE OF AN ACTIVE FIXATION, STYLET DRIVEN LEAD DURING LEFT BUNDLE BRANCH AREA PACING

چکیده
Left bundle branch area pacing (LBBAP) is increasingly being used. Few cases have been reported using a fixed helix, stylet driven lead (Fineline II Sterox EZ, Boston Scientific). Moreover, given the lead’s design, in which electrode not located screw but rather at base of screw, concerns about higher risk inadvertent perforation exist. To describe procedural characteristics LBBAP helix lead. Consecutive patients undergoing Fineline were included. The last consecutive with different leads used as control 2:1 ratio. Before implant, tip was inserted 30 seconds saline to dissolve polyethylene glycol cover. commercially available sheath (SSPC2 or SSPC3, Scientific) and standard technique. After left capture obtained, fully into pushed determine septal perforation. first primary endpoint success. second procedure related complications, including Secondary endpoints included time fluoroscopy time. Between October 2022 November 2022, total 54 (mean age 70.6±11 years, 29.6% female) underwent LBBAP. In 18 There no significant differences baseline characteristics. Procedural success achieved 96% patients, between groups. Patients group had trend towards shorter post QRS duration (113.1±18.9 vs. 125.3±20.6, p = 0.05). Targeted left ventricular endocardial pacing using a steerable introducing guide catheter and active fixation pacing lead.
AIMS Cardiac resynchronization therapy via the coronary sinus (CS) is not always possible. Left ventricular (LV) endocardial lead placement is a potential alternative. The purpose of this study was to assess the feasibility of endocardial LV pacing using a steerable lead introducer and active fixation polyurethane lead. METHODS AND RESULTS Endocardial LV lead placement was attempted in nine p...
متن کاملLong-term temporary pacing with an active fixation lead.
BACKGROUND Ensuring a haemodynamically effective cardiac rhythm is a challenge in patients waiting for pacemaker reimplantation after transcutaneous lead extraction due to an infection of the implanted system. AIM The authors report a retrospective analysis of temporary pacing with an active fixation lead (AFTP) connected to an externalised pacemaker in patients after transvenous lead extract...
متن کاملCardiac pacing in left bundle branch/bifascicular block patients.
The primary concern in patients with bifascicular block is the increased risk of progression to complete heart block. Further, an additional first-degree A-V block in patients with bifascicular block or LBBB might increase the risk of block progression. Anesthesia, monitoring and surgical techniques can induce conduction defects and bradyarrhythmias in patients with pre-existing bundle branch b...
متن کاملRight bundle branch block during transvenous ventricular pacing.
Cardiac pacing through a transvenous catheter located in the right ventricle usually produces a pattern of left bundle branch block (LBBB). When right bundle branch block (RBBB) occurs during apparent right ventricular pacing, a complication (perforation or malposition of the catheter) has usually occurred. In the patient reported here, uncomplicated right ventricular transvenous pacing produce...
متن کاملOvercoming left bundle branch block by permanent His bundle pacing: Evidence of longitudinal dissociation in the His via recordings from a permanent pacing lead
Introduction Narrowing of the QRS and overcoming left bundle branch block (LBBB) with His bundle pacing (HBP) has been previously described. We present the first 2 reported cases of left bundle branch (LBB) delay with evidence of a split His electrogram during unipolar mapping from the tip of the His bundle (HB) lead during pacemaker implantation. These findings suggest that the site of LBB del...
